The decision to undergo fertility treatment is never an easy one. In the past, patriarchal and misogynistic societies often laid the onus of infertility on the woman, and to be “barren” was often the only legitimate grounds for divorce. Much of this stigma – however deeply buried or disguised a “pity” – remains today. It is important for the patient to understand that seeming inability to conceive is not her fault. There are many reasons for infertility that are treatable, correctable, have effect alternatives, or may actually involve the partner.

Regardless of the cause, it’s also important to know of the causes as well as the many treatments available. The first step is to determine the cause behind the inability to conceive. Usually, the problem is ovulation, which can often be corrected easily by relieving stress, changing one’s eating habits, or changing nutritional supplements. There are however many medical causes as well. One of the most common is Polycystic Ovarian Syndrome (PCOS). Women with this syndrome have enlarged ovaries that have several small cysts. Symptoms of PCOS range from mild, such as acne or irregular menstruation, to serious conditions such as high cholesterol and hypertension. Fortunately, this condition is often treatable with medication and a change of diet and exercise habits.

Other causes include tubal conditions, male infertility, and endemetriosis; some causes of infertility that are seemingly inexplicable may be due to insufficient fertilization, slow or arrested cell division, an egg with an overly-thick outer shell, or sperm failure. In order to determine the problem, the office will perform a full fertility work-up that will determine if the problem is with the sperm, the fallopian tubes or the egg or ovulation process. This includes a series of blood tests, an alaysis of semen, and an evaluation of the uterus and ovaries.

Depending on the results of the initial workup, reproductive treatments range from “low-tech,” such as the use of medication, artificial insemination or ovular stimulation, to surgical treatments like diagnostic laparoscopy and tubal reversal (for women who have undergone tubal ligation) and “high-tech” solutions such as in vitro fertilization and preimplantation genetic diagnosis (PGD).

Sam Najmabai, M.D. is a specialist in obstetrics and reproductive endocrinology as well as an assistant clinical professor at the Department of Ob/Gyn of the USC School of Medicine. An active researcher, Dr. Najmabadi has participated in several studies and has published a number of papers on fertility medicine.
